HOW TO ACCESS YOUR DISCOUNTS Step 1: Locate a Provider
To locate a participating provider, visit www.usmedicalbenefits.com to access the on-line provider search or call toll-free 1-800-290-0523.
Step 2: Make an Appointment
Simply make an appointment directly with the provider you’ve chosen. You can also check to see if your current dentist or office is part of the network.
Step 3: At the Provider’s Office
You must show your ID card at the time of visit to receive your discount. You are responsible for the total bill, less the applicable savings, at the time service is rendered.
